Health Certificate  Effective July 9, 2013, Act 59 of 2013 has eliminated the requirement for the Health certificate, except when applying for a Private Academic Certificate.
 
(Completed an approved certification program through PA college/university/education preparation provider with recommendation from that PA college/university/education preparation provider)
Must be submitted through TIMS
 
Out of State PA Certificate Application (Level I) (fee required for each subject area)   
(Completed certification program outside of Pennsylvania)

(Converting Level I to Level II)

Add-On application (fee required for each subject area)  
(Adding on Instructional areas to your current PA Instructional certificate)

(Applying for the Program Specialist - ESL)

(Holds a Letter of Eligibility or Commission Qualification Letter and has been elected by a Board of school directors to serve as a Superintendent or appointed by an Intermediate Unit Board of Directors as an Exceutive Director)

(to be completed only by educators who were certified prior to 1967 and are not on the teacher certification system, and have a copy of their certificate)

Please refer to Certification Fees for the list of fees associated with each application type.  Note:  Application fees are non-refundable.  The fee will be retained by the Commonwealth whether or not the transaction results in the issuance of a certificate.
